If you are a solo practitioner (not part of a group practice) in South Carolina, you can skip down to the section title, “To Register Individual Practitioners 

 

If you are a group practice (or a solo practitioner with a Type 2 NPI number) in South Carolina, you must register your group as an OON practice and the individual practitioners within it, separately. We realize that this may seem like a lot of work, but it's a one-time process that should set your practice up for smooth claim filing going forward. 


To Register a Group Practice 

 

  • Sign into the BCBS of SC portal at https://bcbsscv12.my.site.com/ (you may have to create an account) 
  • Click "Get Enrolled" from the My Provider Enrollment Portal main screen
  • Click "Enroll" in the dark blue box titled "Group Practice Enrollment" 
  • Click the "Continue to Group Enrollment Form" button near the top of the page 
  • You will land on the "Form Information" page, where you must upload a couple of documents by clicking the "Upload Files" button in the "Documents" section 
  • Upload a copy of your business license 
  • Upload a copy of your IRS Verification of Tax ID (Letter 147C or CP 575 E) 
  • Upload a copy of your state certification document (if you are state certified) 
  • After uploading these documents, click the tab that says "Form" to display the form fields. 
  • Some notes about filling out this form 
  • You only need to fill out the required fields, marked with a red asterisk (*) 
  • At the top of the GROUP form, specify "NO" where it asks "Do you wish to be in network?*" 
  • Enter your company's Type 2 NPI number where it asks for your "NPI*", and your company's Tax ID where it asks for your "Federal Tax ID (EIN)*" 
  • Fill out the "Payment Address Information" section, even though you will not be receiving payment from BCBS 
  • For the "Type of Business*" question, select the type that most closely matches your business 
  • For the "Group Provider Type*" question, select the type that most closely matches your business 
  • For the "Provider Type*" question, select the type that most closely matches your business 
  • You should include all the practitioners in your group in the section titled, "List each practitioner that will be providing services at this location" (even though you will have to register each of them again via the Health Professional Application form (see instructions below) 
  • At the bottom of the form, click "Next" to enter the EFT section of the form. 
  • You must complete all required fields in the EFT section of the form, even though you will not receive payments from BCBS as an out-of-network practitioner. 
  • After you fill out the form, you must wait 3 or so minutes for a PDF file to be generated. After this time, please refresh your screen, and the PDFs will be in the Documents section, along with the other documents you uploaded previously. 
  • Your application status will say, "Awaiting Signature." 
  • You’ll now need to download the Electronic Funds Form, print it, sign it, scan it and upload then back to the portal. 
  • A signature is not required on the Group Application 
    • Once all documents have been uploaded, you will see an ACKNOWLEDGEMENT section at the bottom of the screen 
    • If you do not see the “ACKNOWLEDGEMENT” section, click “My Forms” near the top of the page to view a list of your pending forms. 
    • Click on the appropriate form 
    • You should see the “ACKNOWLEDGEMENT” section. 
  • Click the Confirm button to submit the application for review. 
  • Proceed to the next step, below.


To Register Individual Practitioners  

 

  • Sign into the BCBS of SC portal at https://bcbsscv12.my.site.com/ (you may have to create an account) 
  • Click "Get Enrolled" 
  • Click "Enroll" in the light blue box titled "Health Professional Application" 
  • Click the tab that says "Form" to display the form fields. 
  • Some notes about filling out this form 
  • You only need to fill out the required fields, marked with a red asterisk (*) 
  • For the "Effective Date*" question, choose a date in the past that is earlier than the date of service on any of the reimbursement claims you plan to file 
  • For the "Date of Request*" question choose the same date as above. 
  • For the "Federal TaxID Number*" question, use your businesses Tax ID 
  • You will need to upload a copy of each practitioner's National Plan and Provider Enumeration System (NPPES) NPI notification with this application. You can obtain a copy of this notification at the NPPES website. 
  • At the bottom of the form, click "Next" to enter the License Information section of the form. 
  • After completing the required fields in the License Information section, click the blue button at the bottom of the page to "Generate PDF for Signature" 
  • You will be returned to the Form Information screen. Please wait for at least three minutes for the PDF files to generate. After this time, please refresh your screen, and the PDFs will be in the Documents section. 
  • Your application status will say, "Awaiting Signature." 
  • You’ll now need to download the “Health Professional App” and “Authorization to Bill” forms, print them, sign them, scan them and upload then back to the portal.  
  • Once all documents have been uploaded, you will see an ACKNOWLEDGEMENT section at the bottom of the screen 
  • If you do not see the “ACKNOWLEDGEMENT” section, click “My Forms” near the top of the page to view a list of your pending forms. 
  • Click on the appropriate form 
  • You should see the “ACKNOWLEDGEMENT” section. 
  • Click the Confirm button to submit the application for review. 



You will be notified via email from BCBS of SC as your application makes its way through the process.
